Hello Brian (Other), DoC's on the web aren't rare, but they are certainly in the minority. Try the following Google search, enter the following line, as-is, including quotes and you will find a plethora of DoC's.
"Declaration of Conformity" filetype:pdf Then do the same Google Image Search, you'll instantly see what they look like, great if you're seeking 'artistic' inspiration for your own documents.
