This is mainly a curiosity question.

I have used overvoltage categories routinely for many years for LV products
and now it seems to me that there must be something analogous to this on
the MV side of a utility transformer.  Certainly I do not think the OV
Cat Roman numeral system of 60664-1 is appropriate.  Or is it simply
expressed in kV peak and the impedance of the transformer is used to
determine expected impulse on a secondary winding, as in arc-flash
calculations?   Can anyone point me in the right direction to learn more?
