Brian, You might want to change the mode of the counter in the Mesa card to a single channel counter (non quadrature) and hook the index pulse to that channel. Then see if the Index pulse is counting reliably - 1 count per rev. (I think you can change that Mesa card to a single channel counter - check the manual).
