Yeah as I suggested earlier the symptoms seem to suggest loss of communications at the PCI level, which could be a bad 5I25, bad PC power supply (low 3.3v will cause a 5i25 reset, I suggest measuring this) or bad bad/dirty PCI slot in motherboard.
