Gene, Chris, Milosz and everybody who has tried to help, I am reposting this because I screwed up earlier and failed to change the Subject line.
I took the power supply out of the controller cabinet and set it up on the bench. I used a small screw driver to turn the fan a few times, then I turned it on and the fan ran. Like Milosz said it was loud. I turned it off and turned it back on again and the fan ran, Great problem fixed (yeah, right). I put it back in the cabinet and turned it on (silence). So I will take it back out of the cabinet and see if I can get it to run and start reliably after a period of rest. Otherwise it looks like replacing the fan is the answer.
