Thanks for the advice – the number is actually 866-695-0778
And it’s been long enough ago that if I saved a copy of the form and proof I sent, I’d have to turn the place upside down to find it.
I called them and as I feared, there was no indication my request was in the system.
I’m resending the request, as soon as I can find the invoice to print out and he indicated they should take the first page of the minimanual in place of the POP.
