Occasionally I forget that I have a maximum limit on the size of messages to automatically downloaded (in the Accounts settings, under Options). That can cause messages to be downloaded without their attachments. The fine print at the top of the message tells you that it's a partial download and gives you the option to download the whole thing, but a couple of times I've overlooked that and contacted the sender, saying that there was no attachment.
