This sounds quite a bit like the scenario from bug 567581[1] although by using the filename for navigation instead of the image number. Yet, the Lucas' answer on that bug applies here as well in my opinion: Good candidate for a plugin. :) So, please open a new enhancement request for this in our bugzilla, which will keep it slightly more visible than on the mailing list. Technically it's not really a new instance but rather just a new window although it's practically no big difference. And yes, that's how it works at the moment. You also mention the problem with determining the right behaviour, which is one of the reasons we are putting this back all the time. There is a (unreviewed) patch[2] though which adds the possibility to reuse a window from the commandline.
