The MEDIUM ERROR could be it.  Check for :
1)  Proper termination.
2)  Wellness of SCSI card.  Try different slot.  Try different
card.  Make sure it's IRQ is not shared.
3)  Wellness of SCSI cable.  Is it tight?  Is it neat?  Any
missing pieces?
4)  SCSI settings within design parameters?

If the information for a program is weird because it can't get
clean access to the drive then the results are undefined.

Anyway, it's a thought.
