Not necessarily, but likely. You could replace a damaged item with a new copy with a different barcode, for instance.

And, I've seen libraries reuse barcodes of deleted items on new items on a different title/bib record simply because they had extra barcodes laying around. When the long lost/deleted item shows up again and staff try to undelete it or check it in, hilarity ensues.

People don't always do what is most "correct."
