Thanks Milan, this explains it.

Your workaround of Ctrl+2 and then Ctrl+1 appears to bring back the New button 
for
the length of the session, which if true is almost no inconvenience at all. 
This is
what I'll be doing now.

Ctrl+Shift+m is too gnarly a key combo for me to do on a regular basis, so I'm 
glad
Ctrl+2 then Ctrl+1 works.

I'll try Ctrl+2 then Ctrl+1 for a few days to make sure it's not intermittent, 
and
then report back. Thanks so much for letting me know about this.
