After turning on the indexes, you have to make sure and re-generate the indexes, or despite what the configuration says, you will not have indexes for those attributes. Frequently, I have found it's actually quicker to dump and re-add the LDAP database after changing indexing on an attribute, but OpenLdap does come with a tool to re-index, I just don't remember what it is called at the moment.
