I normally add a "LookUp" column at the beginning of the table.

This lookup column then concatenates the criterias.

using this method a normal vlookup will work.

Using your example:

1.) Insert extra column before column A
2.) In the new column A add this in cell A3: =B3&"_"&C3
3.) Drag this formula down to the rest

The result in A3 should be "001_1"

Then you can use vlookup to get the rate:

For instance if you want the rate for series 002 period 1 then use:
=vlookup("002_1",A3:D14,3,false)

(new to the group, so excuse me if I am not following guidelines for
answering, I will as soon as I know them)
