Instead of using the CONCATENATE function with multiple if conditions, better to use a simple helper column with a small formula like this
J2=IF(C2="Y",IF(I2<>"",I2&",","")&C$1,TEXT(I2,""))
And H2=LOOKUP("ZZZZZZZZZZ",J2:N2)
