Where do your indexes reside?

Has anyone deployed indexing on exchange 2000 successfully?  On our smaller
servers (with less then 40 to 80 people) it seems to be ok.  But on our
larger servers, (over 250 users) it uses up huge amounts of CPU and
increases the disk queue depth to 30+.  This causes big performance issues
for our users.

If you have done this, please let me know what you did to get it working.  

Also has indexing been improved on Exchange 2003?
