Ah, well there the crux is how Exchange communicates with Outlook. As Rodger mentioned, new mail notification is accomplished by the Exchange server sending a UDP packet notifying the client of the message.[1] If the client isn't getting the notification, it's being dropped somewhere and a sniffer can likely help you determine where.

As much detail as possible would probably involve reading "Inside MAPI", which is a good book generally, though out of print and not something the typical administrator needs to master for day-today operations.
