It's possible one of your end-users has a virus that's using Outlook to send
out it's spawn.  If you allow internal users to relay through your Exchange
server, it could be any mailing virus (not just those that use Outlook).
I'd disable relaying for internal users and restrict all port 25 traffic out
of your network to your Exchange server only.
