Actually, there is a check every 24 hours that the SiteFolderServer does that checks for duplicate system folders and if any dupes are found, we delete them so only one copy of the folder is there.

It is quite possible that the presence of the second one is confusing Outlook. I have seen a duplicate OAB Version 2 appear after you create a blank Pub.edb and then later restore the original public information store. The blank one starts with its own OAB Version 2 and updates the directory. Then when you restore the original public information store, the original OAB Version 2 folder comes back - and you have two of them. I think I fixed this in the past by running IS/DS Consistency Adjustment on public folders and asked to adjust all inconsistencies. Make sure that they are homed correctly and that they have permissions for Default to be an Editor.
