Yeah, I think this must be it.

She just said that most of these messages are "sent" after 5:00PM. We
get a lot of spam and crap in between 12:00AM and 8:00AM, so I suspect
it's just pushing these messages down the inbox window and out of sight.

I see only two messages in the last two weeks (thanks for the Mailbox
tracker idea!! - should have thunk of that). Both were delivered almost
immediately to her inbox.

I'm beginning to suspect that, because she has so much freaking email in
her
inbox that she didn't see the message.

But I'll be nice and ask her for more info.

Does the sending user have Outlook 2003 with Cached Mode enabled?  We
have a
few users who forget to notice that they are working offline.
Then they create a message and it lays in their outbox until they click
send/receive, or if they don't ever click it the next time the open
outlook
it will perform a send/receive making it late.

Check the "Sent Date/time" and make sure the sender isn't the cause of
the
problem and not the receiver.

I've got one user who's complaining to me that INTERNAL emails are

It seems that these are sporadic and, perhaps, only from one or two

What would cause that type of behavior in Exchange?
