I have posted this before and I will post again, mailfoundry from www.solinus.com is the best money we spent last year. With all due respect to the gentleman who recommended barracuda, I tried them both and mailfoundry is 99% hands off, no false positives, Very little need for Whitelists and excellent tech support. We are not a non-profit and of all the money we spent, this little peice was worth 10 times as much. I have a good friend from our local NT User group who works for a big non-profit and this was a no-brainer for them.
In general the cost of any anti-spam appliance is well worth it the 1st day you dont need to mess with all that junk mail any more. I happen to recommend the mailfoundry.
