We had a similar problem receiving mail through an ISA firewall with its SMTP filtering feature turned on. It was blocking certain SMTP commands, notably XEXCH50, which shouldn't be used over the Internet, but sometimes is. When this happened, our server would just wait for their next command, and theirs would wait for our reply to the XEXCH50, and eventually it would time out with the same delayed delivery error that you're seeing.

I remember seeing a setup tab for EHLO and HELLO parameters but don't remember where that is in the exchange system manager. Could that have anything to do with the 4.4.7 problem.
