I believe that those mailbox rights are added when the mailbox is actually created, i.e., when a message is sent to it or the user first logs on.

When this happens, he can't even log into the new account via OWA using the password he just gave this new account. He then has to manually add all these users and groups (Domain Admins, Exchange Admins, various other groups that should have inherited permissions, etc.). This is a pain. And, of course, this is an intermittent problem.
