Captaris Rightfax gets my vote if there's any integration to do outside of
Exchange -- but their Exchange connector still uses MTA for Exchange 2003
(should change later this year with their Exchange 2007 support).

Interstar LightningFax is a cheaper alternative if you just need MAPI/SMTP
integration for faxing to and from Exchange mailboxes.
