It  probably does, but the tool itself still has a hard limit of 16383
items, even before you try opening the file.  I have several of these
pst files, all less than 1.5G, and they all act exactly the same way.
I'm opening them in Outlook 2003 (and with various tools) but nothing
seems to change the architecture of the file, nor do any of the tools
appear to have the innate ability to work with more than 16K files per
folder.

The tool actually does more than that though, as it supports PST files
larger than 2GB.  So it definitely sounds like something is corrupt with
the format of the PST file to where the utility only recognizes it as
having a limit of 16k items.

Instead of the pruning tool we bought a tool awhile back called Recovery
for Outlook:

This tool splits up 2GB PST files into multiple files, attempts to fix
corruption, removes passwords (very handy when the ps19upg utility
doesn't work) and attempts to recover/undelete items.  Supports all
versions of Outlook.  It's not free, but it has been worth the money
over the past few years.  Especially when the users don't lose a large
chunk of e-mail when pruning.  

We had to cut out 500GB of data before in order to recover a PST file.
Not very nice..

Appreciate the thought, but yeah, I have done that.  Tried deleting some
items, compacting, repairing, nothing I've tried so far seems to reset
the count in the folder.  I'm starting to believe there is no cure.

I don't know of any new tools, but since you're grasping at straws, I
can suggest a couple of fairly hopeless straws, but they might be worth
a try.

First, have you tried moving the entire PSTs into Outlook folders?  Or
moved just the moveable items out, and then see if what's left over
might not fix itself?

Secondly, the ancient Exchange client can  sometimes do things that
Outlook can't.  Pulled my chestnuts out a couple of times.

I've posted this question a long time ago, but am running it by everyone
again in case new tools have surfaced:

We have some archival PST files, which were created by Exmerge from an
Exchange box between 2003 and 2005.  They're not over the 2GB limit, but
they do contain more than the 16,383 item limit in the inbox.  I've
tried every configuration trick I know to get around the file limit,
with no success yet.  I've deleted some files out but nothing seems to
prompt the 'indexing' to reset itself to any items besides those
initially visible.  I had hopes for Microsoft's 2GB 'pruning' tool, but
that tool will not allow you to run it against a PST unless it is 1.9G
or larger.  Does anyone know of a similar pruning tool that might work,
or of any other process to get at the other items in the file?
