We run GFI on a front-end box. Basically, everything coming in from the Internet (or going out to the Internet) gets scanned on the GFI box before it ever goes to Exchange. We've found it not exactly intuitive to configure and upgrade, and the support is sometimes dicey, but the price is right and the product has been working very well for us for both antivirus and antispam for the last 7 years or so. We block most attachment types coming in.
