By default I believe Outlook 2003 has something like a built in 30 second delay when using Cached Mode. This is on purpose so that the Outlook client is not pounding away on the exchange server obsessively looking for new emails. So sometimes emails may take 30 seconds to show up other times it may seem almost instant, again this is normal for cached mode. You can also tweak these parameters to be more aggressive.
