If I remember correctly (pulls down creaky stairs and climbs up into dust filled attic of the brain housing unit), you can set the attachment name, date, subject line of the message you want extracted. Exmerge will create a PST file for each mailbox it scans whether the message is there or not.
If you get the "ILOVEYOU" package from Technet (online), there's a very nice 60-some page word document that explains in very great detail all the inner workings of the utility.
