Go to MMC right click exchange in the tree and properties.  Then click the
directory security and check two boxes Allow anonymous access and basic
authentication.  Also click the virtual directory tab and see if you have
Read, log access and index this directory permissions checked. I think this
is what I did but until I remember try this to see if it works

4.0 set permissions under exchange directory
