Whoopie, I have one. Me. I support 6 sites (soon to drop to 4) with about 5K mailboxes.
I'm not saying don't ever do it, just don't make it a "default". Make it so that the admin has to manually add his NT account to the box each and every time. Make your server update the directory perms every one minute. That's an FAQ. Get EVERYTHING in writing if you are asked to enter anyone else's mailbox. EVERY time. Maybe you need to rethink how you troubleshoot mailbox issues. I almost never have to actually go into anyone's mailbox.
