I had a project some time ago where I had to validate all our custom recipients (thousands of them) and clean out those that don't exist anymore. I used Arclab MailList Controller and it worked quite well. It does not feed directly off Exchange but has a lot of export/import capabilities. And it cleans itself when recipient addresses become outdated.
