I believe it is one per activation of the OOF rule per mailbox... this was sent offline to the Webmaster:
Did you use a different sending address for each of the two OOFs? Exchange sends an OOF only for the first instance of an OOF, so using the same sending address within a short timeframe would result in no OOFs being generated on the second message, for those you send the first message to as well. Try sending your second test message from a different address....
