They can tell. For example if you run EXBPA, it will alert on VMware.

Let say you need PSS, with a spare server you could migrate the VM to physical for troubleshooting. Microsoft tells me that they don't support running Exchange 2007 on a virtual server now, and will only support it for certain roles in the future.
