I've just run across this here too. All clients are Outlook 2007, Exchange 2007 
SP1. I wasn't pay real close attention before it happened here, but we've also 
tried OWA without any solution. In our case, I don't know if the OOF was set 
when the mailbox was migrated, but this is showing up a month after the 
migration.

We've also tried running Outlook with /CLEANRULES, which didn't fix it either.

last resort...disable-mailbox and connect-mailbox?  Other concern would be if 
this is a BES user...there was a bug prior to 4.1.4 SP5 or 6 if I recall 
correctly with OOF.
