You have to install the pear-DB port (/usr/ports/databases/pear-DB).
Also when try to start exim the following happens: /usr/local/etc/rc.d/exim.sh start /usr/local/etc/rc.d/exim.sh: WARNING: sendmail_submit_enable should be set to NO Starting exim.
put endmail_submit_enable="NO" into your /etc/rc.conf Make sure you read /usr/local/share/doc/exim/POST-INSTALL-NOTES as well.
