First,  is it ATAPI?  If so you may want to try these changes in your kernel
configuration:
Under Block Devices:
1) turning off IDE/ATAPI CDROM support
2) enable SCSI emulation support
Under SCSI support
1) enable SCSI CD-ROM support
2) enable SCSI generic support

Then recompile and install the new kernel.  I had the same problem
although my cdrom is internal, I read somewhere that if your kernel sees
an ATAPI cdrom it will load it first and disregard SCSI support.  Also
make sure you have  the line:

append = "hd?=ide-scsi"

in your lilo.conf file .  Obviously replace the ? with the logical drive.
