I am doing something wrong?  Or is something missing in the lists of
packages on the mirrors? (Certainly, the packages are there, e.g.
kernel-2.4.22.18mdk-1-1mdk.i586.rpm)

With the kernel and urpmi, you sometimes need to add more of the version to the command. What worked for me was:
urpmi kernel-2.4.22.18mdk
