On a very busy FarCry site our server admin ran a utility to monitor
sql requests and automatically suggest/set indexes. this was some time
go so cannot remember what indexes/improvements were made.

On other sites I usually start by adding indexes to the
nested_tree_objects, recategories, refcontainers, refobjects - these
have given very significant improvements - in one case just adding
these indexes immediately dropped page load times from 5+ seconds to
sub-second timings.
