FarCry Core has the farUser register* webskins. You can find these in /farcry/core/webskin/farUser/.
I can't really speak as to how well they work as I would normally code something like this by hand, but you could start with those webskins and see if they meet your needs, or copy them to your project and tweak them as you need. If you do use these you will probably have to add the proper view permissions for these as I believe they are restricted out of the box. Otherwise, if you want to write them by hand you could create a form and use the FourQ ORM to create the records or even use a FormTools form and let FarCry do the heavy lifting (as the register* webskins do).
