Status on images is not really used for anything. Images have to be under the webroot in order for it to be visible in the web browser, whether or not its in draft. There's no practical or rather simple way for hiding this content in draft. Files are a little bit different and we have the whole "secure file" shuffle going on as downloads are delivered via an authenticated CFCONTENT stream (hence the need for download.cfm). I think ideally I'd look to remove status from images entirely to end the confusion.
