I'm not going to turn this into a publishing clinic, but by and large most 
people are completely clueless what it takes to put a book together in the 
modern market, and how few people are making how many books.  Not counting 
self-publising on-line only, over 100,000 books a year are published in the 
English language.  There aren't enough hours and if certain deadlines aren't 
met, it's like falling dominos at a publishing house.  No one can say, 'Oh, 
let's stop everything and fix this."  Those days haven't existed since about 
the 1980s.
