Just send it to me in an email in the following format:

{email@address}
{affilition, using linebreaks \\
if necessary to list multiple affiliations}
{any acknowledgment of funding}
{Chapter \ref{chap:foo} % list all your chapters here
No need to scan it or send it by snailmail this time. ;-)
