I have received a patch from a contributor whose employer is not able to sign the institution copyright consent form other than for a limited period of time. (The time limitation is for the period of contribution, not for the period of the validity of the license which is indefinite.) I have therefore prepared a suggestion for a limited version of the institution copyright consent form:
