You originally claimed that your problem is related to multi-channel audio and the WAVEFORMATEX structure (0x2000 vs 00001000-0000-0020-8000-00AA00389B71). The problem - that high-bitrate ac3 in avi muxed by ffmpeg can still be decoded by ffmpeg, but not played by MPlayer and WMP - is perfectly reproducible for stereo sound, just use ffmpeg -i file -acodec ac3 -ac 2 -ab 448k out.avi.
