I have a short but unfortunate answer for you. That version of ffmpeg you're using is soon celebrating its fifth birthday. Honestly, it's very very old, and I don't think developers care for tracking down bugs or checking for known shortcomings. The same is probably valid for the libx264 it makes use of. Please update to a recent version. If you still observe the issue then, you should report again. If not, please do tell us that everything is going okay now.
