Thank you very much for the reply.

I installed ffmpeg 3.3-1~16.04.york1 for Ubuntu 16.04 from PPA. I think it includes all the libraries and command line tools.

I concatenated the initialization file init.mp4 with one of the segments seg1.m4s to new_seg.mp4 using cat. With the help of mp4dump, I could see that all the required information (e.g. [trak] and [trex]) presented in new_seg.mp4.

However, I still got the same errors when I was upconverting new_seg.mp4. The following is the errors returned:
