I filed a bug or two on individual packages where I noticed it, but then realized the bigger issue and filed a meta-bug. Oh, I just realized that you were in on that discussion last December: http://bugs.debian.org/cgi-bin/bugreport.cgi?bug=652011. The ambivalence out there about the / vs. /usr distinction, plus my own personal direction the first half of this year, delayed my intentions to put together my own FHS-utils package together, but recently I've been able to get back on it.
