I don't mean to be impertinent, but you did select the proper radio button at the bottom, no? Occasionally I run into this problem myself. The way I fix it is by noticing whether Measure-Attached or Note-Attached is selected on the radio button at the bottom of the dialog. I find it easy to overlook this option.
