X in this case is short for Xwindows, which is a graphical user interface that can be run on many platforms.

In your case you'd most likely run Apple's X11 (frequently in /Applications/Utilities), which is a particular variant of Xwindows.

The reason that xv can't run by itself is that it wants to display its information on an Xwindows display. This really isn't too different from Mac or Windows apps that require their respective graphic displays to function.
