I have seen this when parts of an old tex installation were interfering with the update process. Unfortunately the tex packages live in many different directories, and they create files after the installation which therefore are not listed in the package file list and not always removed when a new version is installed. Some manual hunting for old tex files, in particular *.fmt files, may be necessary. Check also your private files in ~/Library/texmf and ~/.texmf-config and ~/.texmf-var.
