Or at least that part... If you have a log, can you look at what  
happened during the patch ?
And else, run the last line of the patchscript in the builddir; ie,
cd  /sw/src/transcode-1.0.0-1005/transcode-1.0.0
(or wherever your builddir is), then :
sed -i.bak -e '/#ifndef always_inline/,/^[[:space:]]*$/d' src/ 
transcode.h
and then repeat the diff command :
diff src/transcode.h*

If it still doesn't work, please tell the filesystem on which your  
builddir
lies (UFS, HFS+), any other possible peculiarities, and the output of
"which sed"
(I must have tested all classical variations there, that's why I'm  
looking
for other peculiarities)

And please confirm that it is transcode-1.0.0-1005 you were building...
